The purpose of the survey should drive the survey content

Before you start defining questions to ask in details, take sufficient time to think through the purpose and objectives of the survey. The objectives should always drive the types and content of the questions. The objective also should lead to an action that can be taken based on the answers collected from the survey.

Example:

Objective: Improve our web site

Action: Identify the pages in our website that are difficult to understand and re-design them. Using a survey tool, create questions for each page in the web site that probes whether the respondent has visited the page and if they were able to gather information from the page as intended.
